In this presentation, Dr. Zairys Feliz, Ph.D., LCSW, will guide participants in strengthening their documentation practices to better reflect clinical decision-making while maintaining compliance with ethical, legal, and insurance standards. Attendees will explore how to write clear, concise, and defensible progress notes, treatment plans, and risk assessments, with an emphasis on avoiding common pitfalls that can lead to scrutiny or liability. Participants will also examine how documentation impacts audits, subpoenas, and continuity of care across systems. Emphasis will be placed on practical strategies that balance thoroughness with efficiency, ensuring documentation supports both client care and professional protection. Participants will leave with actionable tools to improve the clarity, accuracy, and defensibility of their clinical records.
