HIGH-RISK & CLINICAL EDGE CASES
Countertransference in High-Attraction or Sexual Tension Cases – 4 CEUs

Overview:
Few topics generate as much discomfort, shame, and avoidance among clinicians as experiences involving attraction, sexual tension, or erotic countertransference within therapy. Therapists may experience unexpected feelings of attraction toward clients, heightened emotional investment, fantasies, excitement surrounding sessions, protectiveness, nervousness, or awareness of relational chemistry that feels difficult to discuss openly. Likewise, clients may express attraction, flirtation, idealization, sexualized interactions, or relational dynamics that create tension within the therapeutic space. Because these experiences can challenge professional identity and evoke fear of judgment, clinicians may avoid consultation or fail to adequately assess how these reactions influence treatment. Unacknowledged erotic dynamics can create blind spots, boundary concerns, enactments, and ethical vulnerabilities that negatively affect both clinician and client care.

In this presentation, Dr. Zairys Feliz, Ph.D., LCSW, participants will examine practical, ethically grounded, and clinically informed approaches for understanding and managing attraction and sexual tension within therapeutic relationships. Attendees will explore how attachment dynamics, personal history, relational patterns, therapist vulnerabilities, transference, and countertransference processes contribute to these experiences. The course will address common therapist reactions including shame, denial, overcompensation, avoidance, excessive self-disclosure, rescuing patterns, and rigid distancing behaviors. Participants will learn practical strategies for increasing self-awareness, recognizing warning signs, utilizing supervision and consultation appropriately, strengthening professional boundaries, and making ethically informed decisions that protect therapeutic integrity. Through case examples and applied discussion, attendees will gain tools for navigating one of psychotherapy’s most sensitive and under-discussed clinical realities.

Objectives:

  1. Identify factors that contribute to attraction, erotic countertransference, and sexual tension within therapeutic relationships.

  2. Recognize therapist reactions and boundary risks that may emerge when erotic dynamics remain unexamined.

  3. Apply ethical and clinically informed strategies for self-awareness, consultation, boundary management, and treatment decision-making.
