HIGH-RISK & CLINICAL EDGE CASES
Differentiating Trauma vs Personality Adaptations in Adults – 4 CEUs

Overview:
Adult clients frequently present with interpersonal patterns, emotional responses, and behavioral traits that can be difficult to conceptualize clinically. Chronic emotional dysregulation, fear of abandonment, perfectionism, relational instability, emotional detachment, hyper-independence, people-pleasing, rigid control, or intense reactions may be interpreted as personality characteristics, trauma responses, or some combination of both. Distinguishing trauma-related adaptations from enduring personality patterns can be clinically challenging, particularly when developmental trauma, attachment disruptions, chronic stress, and long-standing coping strategies have shaped the way individuals function over time. Oversimplifying these presentations may result in inaccurate conceptualization, stigma, or treatment approaches that overlook underlying causes.

In this presentation, Dr. Zairys Feliz, Ph.D., LCSW, participants will examine practical and trauma-informed approaches for differentiating trauma-related adaptations from broader personality functioning patterns in adults. Attendees will explore how attachment experiences, chronic developmental stress, relational environments, and nervous system adaptations influence personality development and symptom presentation across the lifespan. The course will address common areas of clinical overlap including emotional reactivity, interpersonal difficulties, dissociation, avoidance, identity instability, perfectionism, dependency, and defensive coping patterns. Participants will learn strategies for improving assessment processes, reducing premature conclusions, recognizing contextual influences, and developing more nuanced case conceptualizations that avoid pathologizing adaptive survival responses. Through case examples and applied discussion, attendees will gain increased confidence in navigating one of outpatient practice’s most clinically complex diagnostic and treatment challenges.

Objectives:

  1. Identify areas of overlap and distinction between trauma-related adaptations and broader personality functioning patterns.

  2. Recognize the influence of developmental experiences, attachment disruptions, and chronic stress on adult symptom presentation.

  3. Apply trauma-informed assessment and case conceptualization strategies that support accurate and ethically grounded clinical decision-making.
