Overview:
Therapeutic ruptures can occur at any point in treatment, but ruptures that emerge later in the therapeutic process often carry unique complexity and emotional intensity. After months or years of developing trust, progress, and relational investment, therapists and clients may experience sudden shifts that threaten treatment continuation. Clients may become withdrawn, angry, avoidant, critical, guarded, resistant, or unexpectedly express desires to terminate treatment. These moments can feel especially destabilizing for clinicians who may experience confusion, defensiveness, self-doubt, frustration, or urgency to repair the relationship. When not recognized and addressed effectively, late-stage ruptures can result in premature termination, unresolved relational injuries, and disruption of meaningful therapeutic progress.
In this presentation, Dr. Zairys Feliz, Ph.D., LCSW, participants will explore practical and relational approaches for identifying and responding to significant therapeutic ruptures that arise during later stages of treatment. Attendees will examine common contributors to late-stage ruptures including attachment dynamics, dependency concerns, treatment plateaus, perceived misunderstandings, unspoken dissatisfaction, shifts in goals, transference and countertransference reactions, and therapist misattunement. The course will address subtle indicators that rupture may be developing before clients explicitly discuss termination and provide strategies for approaching difficult conversations without increasing defensiveness or withdrawal. Participants will learn practical interventions for strengthening repair attempts, addressing therapist reactions, navigating treatment impasses, and supporting thoughtful termination decisions when repair is not possible. Through clinical examples and applied discussion, attendees will leave with increased confidence in managing one of psychotherapy’s most emotionally charged and clinically significant moments.
Objectives:
Identify common relational and clinical factors that contribute to late-stage therapeutic ruptures and treatment instability.
Recognize subtle warning signs that may indicate rupture processes are developing before termination is discussed.
Apply evidence-informed communication and intervention strategies to support rupture repair and clinical decision-making.
Scope and Limitations Disclosure
This course provides licensed and pre-licensed mental health professionals with an overview of therapeutic rupture processes and intervention strategies related to late-stage treatment challenges. Participants will learn practical approaches to rupture identification, repair, and treatment planning. This training does not provide certification in specific therapeutic modalities or comprehensive instruction in advanced relational psychotherapy models. Participants are expected to apply course content within their professional scope of practice, ethical standards, and clinical judgment.

Cancellations made 7 days or more prior to the scheduled training date are eligible for a full refund minus a $25 administrative fee, or participants may elect to receive a full credit toward a future training.
