Overview:
Suicide risk is often associated with visible symptoms of depression, emotional distress, or impaired functioning. However, many individuals experiencing significant suicidal thoughts continue to maintain careers, relationships, academic performance, caregiving roles, and outward appearances of stability. High-functioning clients may present as successful, composed, humorous, engaged, or achievement-oriented while concealing severe emotional pain, hopelessness, and suicidal ideation. These presentations can create dangerous blind spots in assessment and may lead clinicians to underestimate risk based on appearance, productivity, or assumptions about functioning.

In this presentation, Dr. Zairys Feliz, Ph.D., LCSW, participants will examine the unique challenges involved in identifying suicide risk among clients who do not fit traditional clinical expectations of depression or crisis. Attendees will explore how perfectionism, chronic over-functioning, emotional masking, cultural expectations, caregiver roles, trauma histories, and internalized stigma may contribute to concealed distress and underreported suicidality. The course will address common cognitive biases and clinical assumptions that interfere with risk assessment, including the mistaken belief that successful or highly functioning individuals are inherently lower risk. Participants will learn practical strategies for asking more effective assessment questions, recognizing subtle warning signs, strengthening documentation, and conducting comprehensive suicide evaluations even when clients appear emotionally regulated and outwardly successful. Through clinical examples and applied discussion, attendees will leave with greater confidence in identifying risk that may otherwise remain hidden.

Objectives:

  1. Identify factors that contribute to concealed suicide risk among high-functioning clients and individuals who do not present with traditional depressive symptoms.

  2. Recognize subtle warning signs, cognitive biases, and assessment pitfalls that may lead clinicians to underestimate suicide risk.

Scope and Limitations Disclosure
This course provides licensed and pre-licensed mental health professionals with an overview of suicide risk assessment among high-functioning clients with nontraditional presentations. Participants will learn assessment considerations and intervention strategies related to suicide risk identification and clinical decision-making. This training does not provide crisis certification, emergency psychiatric training, or specialized instruction in comprehensive suicide intervention models. Participants are responsible for applying course material in accordance with their professional scope of practice, ethical obligations, and state-specific regulations.
